Any monster of level 120 or higher can drop it. Equipment from a level 120 monster is picked at random from 79 pieces, this one included, each as likely as the others.

Its effects count every hero’s level of it together: with all 6 heroes at level 10, 60 in all, MDEF + 40 becomes MDEF + 160. Disassembled, it gives Maple Logs, like every Uncommon armor. Once you’ve had one, you can craft more from the equipment dictionary when the Blacksmith reaches level 40: 60,000 Maple Logs at first, more as your heroes level it.
