It brings a fainted Nexomon back with 250 HP, or its max HP if that's lower. It can't save one that Custom Mode's permadeath has fainted for good.
You can use it from your bag in battle, where it's your action for the turn, or from the menu outside battle. The game only lets you use it on a Nexomon that needs it.
