Getting afflictions

The Healer offers you potions. Drinking one heals you, but it also gives you an affliction. The bigger the potion, the worse the affliction: it has as many skulls as the potion's size, up to 3. Water heals you without giving you one.

  • Water No affliction
  • 1-skull potion 1 skull
  • 2-skull potion 2 skulls
  • 3-skull potion 3 skulls
The Healer’s potions, and the affliction each one gives you.

With the Third Kidney skill from surgery, a 2-skull or 3-skull potion gives you one skull less.

Afflictions also come from:

  • The Trainer: from the Legionnaire difficulty on, as its description says, training too hard can give you one.
  • The Surgeon: some operations risk one, with a 1-in-3 chance of a 1-skull or a 3-skull affliction or an unknown one as their price, and an amputation always leaves one.
  • Side jobs: fail one and the player who took it gets the affliction they were threatened with.
  • The Owner, who on some nights can make one of you, or all of you, suffer an affliction.
  • Clever Monkey, which starts you off with a random 2-skull one.

Levels and skulls

An affliction has up to 3 levels, and its level is the number of skulls it shows: one skull at level I, three at level III. The game calls the levels Minor, Moderate and Serious.

Get an affliction you already have and it goes up by the new one’s skulls instead, but never past its highest level. Each level has its own effect, and each affliction’s page lists them.

Curing afflictions

A cure takes skulls off an affliction, and once it has none left it’s gone.
